Metoclopramide Hydrochloride Soft Capsule are spherical, transparent and yellow colors containing 10 mg of active ingredient.

mechanism of Effect :
-    The effect of dopaminergic inhibiting: mechanism of the real effect in the treatment of delayed gastric emptying and peristaltic stimulant effect is unknown. However it is thought that, metoclopramide inhibits dopamine that caused relaxation of gastric smooth muscles and therefor increases gastrointestinal smooth muscle cholinergic responses.
This medicine, increases the activity of antral, intestinal transit and accelerates the emptying of the stomach by preventing the stomach muscles relaxation. At the same time, this effect is associated with relaxation of the upper part of the small intestine and As a result, synchrony between the stomach and the antrum of the stomach and upper part of small intestine improves.
It decreases of coming back into the esophagus by increasing the lower esophageal sphincter pressure and improves esophagus acid excretion by increasing the amplitude of peristaltic contractions of the esophagus.
-    Antiemetic Effect: interaction this drug with dopamine, increases threshold regional activity of trigger receptors and reduces stimulate transfer of visceral afferent nerves.
-    Other effects:  this drug increases prolactin secretion stimulating.
Metoclopramide Capsules Indications :
-    Treatment of gastric paralysis: It is consumed for relieve of acute and regressive symptoms of stomach paralysis caused by diabetic gastric.
-    In the treatment of nausea and reflux of gastric contents into the esophagus: it is used In adults for symptomatic short term treatment of stomach burn and esophagitis reflux caused by delayed gastric emptying.
-    In the treatment of vascular headache (migraine): it has been used In prevention to stomach cramps and vomiting caused by migraine and for Increase the absorption of oral Pain medication for the treatment of migraine.
-    In the treatment and control long and persistent hiccups.
-    Treatment of breast-feeding disorders.
Before taking this medication, consult your physician or pharmacist in the following cases :
-    In the case of sensivity to Procaine and procainamide (patients who are sensitive to procaine and procainamide, may also be sensitive to this drug).
-    In the case of sensitivity to metoclopramide
-    In the case of sensitivity to protective Material, Colors and gelatin.
-    If you take other medicines, especially depressant and products containing alcohol.
-    If you have other diseases, especially epilepsy, stomach bleeding, mechanical obstruction or perforation of the stomach digestive tract, pheochromocytoma or severe damage kidney function.
Use in pregnancy : extended studies have not been done in humans, but studies in animals have not shown adverse effects on the fetus. This medicine with in pregnancy drugs, belong to( B ) group and its use is prescribed by a licensed physician.
Use during lactation : effects in humans have not been recorded, however, the benefits of the medicine must be weighed against the risks, because metoclopramide diffused to   breast milk.
Pediatric Use : Dosage for children has not been set. Use of this drug in patients weighing less than 50 kg is not recommended.
Use in the Older people : extrapyramidal effects, especially Parkinsonism, Are likely to be created in older people during normal use or high doses for a long time.
General tips for proper use of drugs :
-    Meal Drug with regular intervals and complete the course of treatment
-    Do not use more than the recommended dose
-    Meal it at bed time or 30 minute before food.
During medication note the following :
-    be care when driving or operating with machinery that requires complete alertness.
-    Avoid Products containing alcohol and depressant drugs (CNS).
Used with caution in the following cases :
-    If you have epilepsy, stomach bleeding, mechanical obstruction or perforation of the gastrointestinal tract, pheochromocytoma, asthma, depression, Parkinson's disease, kidney disorders, allergy to metoclopramide, procainamide and procaine or cancer or breast tumors
Side effects :
In addition of desired therapeutic effects, each drug may cause some side effects, too, Although this effects is not seen in all people, but if you notice any of the following side effects, consult with your doctor or pharmacist :
-    Complications that require medical attention and usually occur at least one year after treatment and may persist after discontinuation of the drug.
-    Agranulocytosis (fever, chills, sore throat, general feeling of tiredness or weakness)
-    The effects of cardio - vascular, especially  blood pressure drop (dizziness, fainting)
-    Increased blood pressure (dizziness, severe or persistent headache)
-    Extrapyramidal effects, dystonia (spasm of the muscles of the face, neck and back), sudden movements or tics, the torsion of the body, inability to move the eyes, weakness of the arms and legs.
-    Extrapyramidal effects related to Parkinson's: difficulty in speaking and swallowing, loss of balance control, mask-like face, staggering, trembling and shaking of hands and fingers.
Start of the symptoms may vary from several weeks to several months and are reversible upon discontinuation.
-    Late dyskinesia (fast or spiral movements of the tongue, uncontrolled chewing movements, slow-moving legs and knees).
-    Note: extrapyramidal effects, may occur in treatment usage amount of different age groups But occur in the most of children and teenager in the case of high consumption in the prevention of chemotherapy-induced vomiting.
High consumption values :
-    Excitement (unusual disturbances, restlessness or irritability)
-    Feelings like fear, signs of restlessness in the legs (inability to feel pain or discomfort in the legs or feet)
-    Note: These symptoms may occur within minutes after receiving the high dose and may continue for 2 to 24 hours.
If symptoms continue or harassment need medical attention are :
-    Common and high frequency: Diarrhea with high consumption, drowsiness, restlessness, unusual fatigue or weakness.
-    Common and low frequency or rare: Sensitivity or mastitis, menstrual irregularities, constipation, dizziness, insomnia (difficulty falling asleep), depression, stimulates prolactin (increase flow of breast milk), nausea, rash, unusual dry mouth, unusual irritability.
Administration and Dosage :
In adults and adolescents:
-    In the treatment of nausea or reflux of gastric contents into the esophagus (reflux)
-    10 mg (one capsule) 30 minutes before each meal or at bedtime for up to 4 times per day or 10 mg 30 minutes before starting the likelihood of reflux symptoms.
-    Note: The symptoms of GERD (reflux) may be removed with taking 20 mg (two capsules) before irritation start.
In the treatment of gastric paralysis caused by diabetes:  
10 mg (one capsule) 30 minutes before each meal or at bedtime for up to 4 times per day or 10 mg 30 minutes before starting the likelihood of reflux symptoms.
In hiccup treatment :
- 10 to 20 mg 4 times daily for 7 days
Note: In patients with impaired renal function, the initial dose should be reduced approximately 50%.
The usual dosage in adults:
- 0.5 milligrams per kilogram of body weight per day.
For Children :
taking the drug Dosage in children, according to doctors recommend and patients with weighing less than 50 kg is not recommended.
Drug interactions :
If you take any of the following drugs before taking metoclopramide capsules, consult your doctor or pharmacist:
-    Products containing alcohol, anticholinergic drugs, opioid drugs, Bromo criptin, cimetidine, central nervous system depressant drugs, cyclosporine, digoxin, levodopa,  succinylcholine and enzyme inhibitor
Poising :
In the case of taking drug more than the recommended dose, stop treatment  and transfer the patient immediately to medical centers.
Symptoms of poisoning include confusion, severe drowsiness, severe extrapyramidal symptoms, seizures.
Storage Condition :
-    Keep drug below 25 oC (preferred 15-25 oC) and away from light and moisture. Keep in original package.
-    Keep away from children.
-    Do not use expired drug.
